As the country focuses on the restructure of the U.S. health care delivery system, nurses will continue to play an important role. It is expected that more and more nursing jobs will become available out in the community, and less will be available in acute care hospitals.

Write a speech of 500-700 words to educate nurses about how the practice of nursing is expected to grow and shift. Include the concepts of continuity or continuum of care, accountable care organizations (ACO), medical homes, and nurse-managed health clinics.

Assessment-2 consist of (1) a case study report and (2) a rehabilitation resource.

The assignment requires you to (1) undertake a case study with an older person and his/her family carer to develop a rehabilitation plan using one (1) evidence based rehabilitation intervention resource and(2) develop one (1) rehabilitation resource which older people family carers and other health care practitioners could use to address the rehabilitation goal you identify in your case study.Document Preview:
